NEW DELHI: A 45-year-old man from Himachal Pradesh suffering from advanced liver cirrhosis recently underwent a successful endoscopic procedure at Sir Ganga Ram Hospital. The patient, hailing from Shimla, had a complex dual obstruction in his food pipe, which was effectively treated during the procedure, addressing two distinct oesophageal conditions.
The man was diagnosed with chronic liver disease (cirrhosis), a progressive condition that leads to liver shrinkage. This had caused complications such as the development of large, twisted blood vessels, known as oesophageal varices, at the junction of the food pipe and stomach, posing a significant risk of severe oral bleeding.
Dr. Shivam Khare, Consultant in the Gastroenterology Department at Sir Ganga Ram Hospital, elaborated on the challenges faced during the treatment. The options included a high-risk surgical procedure or an endoscopic approach, both complicated by the presence of swollen, dilated veins at the gastro-oesophageal junction. To ensure a safe and effective outcome, the medical team worked closely with interventional radiologists Dr. Arun Gupta, Dr. Ajit Yadav, and Dr. Raghav Seth to develop a comprehensive treatment plan.
Dr. Anil Arora explained the peroral endoscopic myotomy (POEM) procedure, which involved carefully creating a mucosal entry, developing a submucosal tunnel, performing a diverticulum myotomy, and addressing the lower oesophageal sphincter. Special precautions were taken throughout the procedure to prevent bleeding from the varices, ensuring the treatment was both safe and successful.
A multidisciplinary team at Sir Ganga Ram Hospital carried out a highly specialized endoscopic procedure to treat the dual oesophageal conditions. Endoscopy is a minimally invasive technique that uses a flexible tube equipped with a camera and tools to visualize and treat conditions within the gastrointestinal tract.
The procedure was meticulously planned to address both the oesophageal varices and the obstructions in the food pipe simultaneously. According to the medical team, the treatment not only alleviated the immediate risk of severe bleeding but also restored the patient’s ability to eat and drink comfortably.Following the successful procedure, the patient is reported to be recovering well. The risk of severe bleeding has been significantly reduced, and his oesophageal function has improved, allowing him to resume a normal diet. Doctors have advised him to continue regular follow-ups to monitor his condition and avoid potential complications in the future.
